Revealed: Donegal HSE data breaches in 2025

PATIENT notes were misfiled twice in a third party file in the maternity department of Letterkenny University Hospital.

The incidents were two of 33 data breaches recorded by the HSE in Donegal in 2025.

The Information was released to the Donegal News under the Freedom of Information Act.

Of the data breaches, ten related to LUH, while 12 data breaches were recorded in Primary Care.

Five data breaches were recorded in Disabilities, four in Mental Health, one in Older Persons, and one in Quality Safety and Service Improvement.

The majority of the data breaches recorded at LUH occurred in the maternity department, according to the FOI. A patient identifier label inadvertently given to a third party, and an appointment letter containing personal and health data of a service user inadvertently issued to an unintended third party were among the breaches.

In Primary Care, within Child Psychology Services, an email containing personal health data of a service user was inadvertently emailed to a third party.

Elsewhere in Primary Care, at the dental department of St Conal’s Hospital, personal details of a service user were inadvertently shared verbally with a third party.

Of the four data breaches in Mental Health, three occurred within Child and Adolescent Mental Health Services (CAMHS). A patient file that could not be located and a letter containing personal and health data of a service user inadvertently posted to and viewed by an unintended third party were among the breaches.
