Programme for adults with long term health conditions to be held in Bundoran

The Living Well Programme is a key evidence-based resource of the Self-management Support Programme across Community Healthcare Cavan Donegal Leitrim Monaghan Sligo (CH CDLMS).

Living Well supports adults, 18 years and over, living with long term health conditions such as Diabetes, COPD, Asthma, heart conditions, Multiple Sclerosis, Parkinson’s Disease, Arthritis and chronic pain to develop self-management capacity and to optimise their health outcomes and quality of life.

Self-management is what a person does every day to manage their long term health condition. This may include making choices to improve their health such as becoming more active, eating more healthily and stopping smoking. It may also include more complex tasks such as taking medication, monitoring symptoms, coping with the emotional aspects of the condition and communicating with healthcare professionals.

The Living Well Programme is also beneficial for those caring for people living with a long term health condition.

The programme is available online and in-person.  Participants on the online programme are supported to access and use the virtual platform.

All programmes are free to access.

The next programme will commence on Monday July 24 at 10.30 am in Bundoran Parish Hall, Bundoran, Co. Donegal.

According to Brid Kennedy, Chronic Disease Lead, Health & Wellbeing CH CDLMS,  “We would like to encourage adults living with long term health conditions and their carers to avail of this programme. People across our five counties who have completed the programme have provided a wealth of positive feedback about the impact of the programme on their lives.

“This positive feedback is supported by international and Irish research studies which have shown the effectiveness of the programme in terms of improving quality of life, physical activity and confidence levels for self-managing health”.
